Research Survey — Participant Information & Consent Notice
This notice tells you, in plain English, what we ask you in our research surveys, what we do with your answers, and how you can stop us from using them at any time. It is provided to satisfy Articles 13 and 7 GDPR (information to data subjects + conditions for valid consent).
1. What the research is for
We are building Tetrik, an ML tool that reduces void space in e-commerce packaging to cut materials cost and CO₂, and to help operators comply with the EU Packaging and Packaging Waste Regulation (PPWR, Regulation (EU) 2025/40). The surveys gather operational reality from the industry — pain points, current workflows, regulatory readiness — so that what we build solves a real problem rather than an imagined one.
2. Who can take part
Adults (16+) working in packaging operations, supply chain, sustainability, e-commerce, manufacturing, fulfilment, or related roles at organisations established in the EEA, UK, or Switzerland. Participation is entirely voluntary.
3. What we ask
Typical questions cover:
- your industry, company size, country, and role (no name required);
- the volume and type of orders you ship;
- current packaging workflow, void measurement, filler use;
- PPWR awareness and readiness;
- optional open text comments;
- optional email address — only if you want a copy of the results or want to be contacted about the pilot.
We do not ask for special-category data (health, religion, political views, etc.) and we do not need any consumer or end-customer personal data.

5–10 minutes.
5. Legal basis
Your explicit, freely-given, specific, informed and unambiguous consent under Art. 6(1)(a) GDPR, given by ticking the consent box and submitting the form.
6. Opt-in — how consent is given
- The consent checkbox in the survey is not pre-ticked.
- Submitting without ticking it is technically blocked.
- You may submit the survey without providing an email; in that case your responses cannot be linked back to you and clauses 7 and 8 below are effectively academic.
7. Opt-out / withdrawal — how to stop us using your answers
You may withdraw consent at any time, in any of these ways:
- Email hello@tetrik.ai with the subject "SURVEY OPT-OUT" and the email address you used.
- Reply "STOP" to any email we send you in connection with the survey.
- Click "unsubscribe" at the bottom of any results or pilot email.
On withdrawal:
- we delete identifiable survey responses tied to your email within 14 days and confirm deletion in writing;
- aggregated, anonymised statistics derived before withdrawal may be retained (they no longer identify you);
- withdrawal does not affect the lawfulness of processing before withdrawal (GDPR Art. 7(3)).
8. Retention
- Identifiable survey responses: maximum 24 months from submission, then deleted or anonymised.
- Anonymised statistics: may be kept indefinitely and published.
- Records of consent (timestamp + version of the notice): kept as long as the underlying response, plus 3 years to evidence compliance.
9. Sharing
- We do not sell or rent your data.
- We do not share identifiable responses with third parties.
- We may publish anonymised, aggregated insights in white papers, blog posts, and the pitch deck.
- Our sub-processors (Google Workspace for Sheets/Apps Script; the email tool we use; Netlify for hosting) process the data on our instructions only. See tetrik.ai/legal/sub-processors.
10. International transfers
Responses are stored in EU regions where available. Where a sub-processor is US-based (e.g. Google LLC, Netlify Inc.) we rely on the EU-US Data Privacy Framework and the EU Standard Contractual Clauses (2021/914) with the UK Addendum and Swiss adaptations as relevant. See Section 4 of the Privacy Policy.
11. Your rights
You can ask us to (a) confirm what data we hold about you, (b) correct it, (c) delete it, (d) restrict or object to processing, (e) port it, and (f) withdraw consent. Email hello@tetrik.ai. We respond within one month. You also have the right to lodge a complaint with the Dutch Autoriteit Persoonsgegevens (or the supervisory authority in your country).
12. Risks and benefits
- Risks: minimal. The data we collect is operational and professional, not sensitive.
- Benefits: you help shape a tool designed to cut packaging waste and CO₂ across European e-commerce, and — if you opt in — you get early access to the results and to the pilot.
13. Funding and conflicts of interest
This research is funded by AmPhi Labs B.V. i.o. internally. There are no external sponsors. The findings inform Tetrik's product roadmap.
